Diabetic Obesity Weight Loss Program
Being overweight or obese is the leading contributor to developing diabetes. Our Diabetic Obesity
program incorporates our Seven Essential Elements™ through comprehensive evaluations, team-based support,
structured learning and fitness environments plus a customized level of medical care designed specifically
to address the special needs of the diabetic patient. Think of our standard program with additional features
like counseling from a diabetes educator and a detailed action plan to work in conjunction with your family
doctor to improve your health. Prices for this program vary.
This unique program includes Health Restoration Blueprint or Change for Good Program plus:
- One-on-one consults with a Certified Diabetes Educator
- Self-care and self-triage classes person aimed at educating individuals on how best to choose
personal care for minor illnesses and how to decide when and where to seek professional services.
- Self-management classes aimed at helping individuals take responsibility for managing the daily
care of their chronic illness
- Nurse care management with a Registered Nurse who coordinates and integrate all of their
healthcare needs, including inpatient and outpatient resources
- Hemoglobin A1-C measurement and monitoring
- Lipid review to evaluate the amount of total cholesterol, triglycerides, high-density lipoprotein
(HDL) cholesterol, low-density lipoprotein (LDL) cholesterol, very-low-density lipoprotein (VLDL)
cholesterol, the ratio of total cholesterol to HDL, and the ratio of LDL to HDL.
- Influenza Vaccination
- Eye & Feet examination
- Electrocardiogram
- Stress echocardiogram is performed to check the overall functionality of the heart and to help
determine if there is any heart disease present.
- Peripheral vascular screening and examination including an ankle brachial index (ABI) test
- Hypertension education and management
- Care advocacy such as provider network accessibility, physician-patient communication problems,
availability of specialist referrals, health plan customer service complaints, claims problems and
collection agency issues and quality of care concerns.
- Coaching—including taking control in medical visits by asking more pointed questions, becoming
more effective in health information gathering, learning skills such as medical journal keeping
and medical visit agenda planning, and how to maximize physician’s visits.
- Smoking cessation classes
ILWM also provides manuals, tools, processes, protocols and other materials needed to support
patient-specific programs for insulin pump therapy and diabetes treatment during pregnancy.
